Welcome to our practice, when you first visit our dental office, you’ll find us in the Boulevard Business Park at 1920 Palm Beach Lakes Blvd, Suite 108. To get to our office, one can take I-95 exit 71 (Palm Beach Lakes Blvd) and head southwest. Our medical plaza is located between Bank of America and La Quinta Inn. There is plenty of parking and is wheelchair accessible.
Before you arrive at our office for your first appointment, we invite you to complete your new patient information forms. By completing this documentation prior to your visit, you’ll help us get straight to the important part of your appointment, partnering with you to develop a plan for ongoing oral health care. If you have questions about your policy or treatment options, please don’t hesitate to ask. Bring along any policy documents we can review to help you understand your coverage. We accept payment at the time of service in the form of cash, check, and major credit card. If you need help covering the out of pocket costs of your dental care, we partner with CareCredit to provide low and no interest payment plans for qualified patients.
